Can I use the -n to distinguish knowing the son from knowing the doctor?
(2) La filon de la kuracisto, kiun vi konas, alvenos morgaux.
Or:
(3) La filo de la kuraciston, kiun vi konas, alvenos morgaux.

Nope. Words after "de" do not generally take the accusative, and "filon" would not be correct because is the subject of the sentence. You could distinguish the two meanings as follows: Tiu filo de la kuracisto, kiun vi konas, alvenos morgaŭ. La filo de tiu kuracisto, kiun vi konas, alvenos morgaŭ.Okay. Thanks.
